Many factors such as pregnancy, breast feeding, weight loss, loss of skin elasticity and the force of gravity can take their toll on a woman's breasts. Her breasts often lose their shape and firmness and begin to sag because the skin loses its elasticity.

Breast lift, or also known as Mastopexy, is a very common surgical procedure among women to raise and reshape sagging breasts. Breast lift can also reduce the size of the areola, the darker skin surrounding the nipple.

If patient's breasts are small or have lost volume, breast implants inserted in conjunction with a breast lift can increase both firmness and size.

A good candidate for breast lift surgery is the patient who has one or more of the following conditions:
           - Breasts that are pendulous, but of satisfactory size
           - Breasts that lack substance or firmness
           - Nipples and areolas that point downward, especially if they are positioned below the
             breast crease

Sometimes these conditions may be inherited traits. In certain cases, the breasts may have developed differently so that one breast is firm and well positioned while the other is not. There may be differences in the size of your breasts as well as their shape. Breasts that are large and heavy can be lifted, but the results may not be as long-lasting as when the procedure is done on smaller breasts.

Breast lift can be performed at any age. Pregnancy and breast-feeding may have significant and unpredictable effects on the size and shape of patient's breasts. However, many women decide to undergo breast lift surgery before having children and feel that they can address any subsequent changes later. Since the milk ducts and nipples are left intact, breast lift surgery usually will not affect your ability to breast-feed.

Plastic Surgeon will examine patients' breasts and measure them while they are sitting or standing and will discuss the variables that may affect the procedure such as age, size, shape of breasts, the condition of breast skin, whether an implant is advisable and where the nipple and areola will be positioned: they'll be moved higher during the procedure, and should be approximately even with the crease beneath your breast.

Types of Anesthesia
Breast lift will be performed in Bangkok Phuket Hospital and under general anesthesia, which means that patient will sleep through the operation.

Pre-operative care
The patient will be admitted to the hospital on the day of surgery. The patient needs to empty her stomach at least 6-8 hours before operation. If patient uses Aspirin or any anticoagulant drugs regularly, she has to stop at least 1 week before operation. Chest x-ray, EKG, routine blood tests will be done on the admission day. If the patient has any allergy history, she has to inform the doctor before surgery. The patient is admitted two nights in the private room after operation and the patient can be discharged from the hospital in the next day.

The Surgery
The incision outlines the area from which breast skin will be removed and defines the new location for the nipple. When the excess skin has been removed, the nipple and areola are moved to the higher position. The skin surrounding the areola is then brought down and together to reshape the breast. Stitches are usually located around the areola, in a vertical line extending downwards from the nipple area (vertical scar mastopexy, superomedial pedical technique, developed by Elizabeth J. Hall-Findlay).

This operation will have scar around the areolar and vertical scar from areolar down to the inframammary fold (lower fold of the breast) and in some cases may have a short horizontal scar in the breast fold if skin is very excess. If patient can accept that this operation have scar then doctor will do this operation.

It is important to remember that breast lift scars are extensive and permanent. They often remain lumpy and red for months, then gradually become less obvious, sometimes eventually fading to thin white lines. Fortunately, the scars can usually be placed so that the patient can wear even low-cut tops.

Patients should also keep in mind that a breast lift will not keep you firm forever because the effects of gravity, pregnancy, aging, and weight fluctuations will eventually take their toll again. Women who have implants along with their breast lift may find the results last longer.

Patients' satisfaction with a breast lift is likely to be greater if patients understand the procedure thoroughly and if your expectations are realistic.

The different between Breast Lift only and Breast Lift with Implant Augmentation is the fullness in the upper pole of the breast, the implant will help to make the upper part of the breast look more full than breast lift alone. For some patients, breast lift alone is still not get the good shape of the breast because the upper part of the breast still flat and loss of fullness to make the breast look like her breast when she was 20 years old, so when we augment with implant, it will make the upper pole full and get the perfect shape.
