Women with overly large, pendulous breasts may experience both health and emotional problems. In addition to self image issues, women may also experience physical pain and discomfort. The weight of excess breast tissue can impair the ability to lead an active life. The emotional discomfort and self-consciousness often associated with having large pendulous breasts is as important an issue to many women as the physical discomfort and pain. A variety of medical problems caused by the excessive weight, from back and neck pain and skin irritation to skeletal deformities and breathing problems. Bra straps may leave indentations in their shoulders and unusually large breasts can make women feel extremely self-conscious.
Breast Reduction Surgery, also known as Reduction Mammoplasty, is a procedure to removes excess fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer.
It involves a procedure to counteract drooping of the breasts and also reduce the size of the areola, the darker skin surrounding the nipple. The aim of surgery is to give the woman smaller, better-shaped breasts in proportion with the rest of her body and to alleviate the discomfort associated with overly large breasts.
Candidates for Breast Reduction
Patients who undergo breast reduction surgery frequently are seeking relief from physical symptoms caused by the excessive weight of large breasts. Breast reduction usually can solve these problems as well as improve the size and shape of your breasts. Following breast reduction, your breasts will be more proportional to the rest of your body, and clothes will fit you better.
You may be a good candidate for breast reduction if you have one or more of the following conditions:
- breasts that are too large in proportion to your body frame
- heavy, pendulous breasts with nipples and areolas that point downward
- one breast is much larger than the other
- back, neck or shoulder pain caused by the weight of your breasts
- skin irritation beneath your breasts
- indentations in your shoulders from tight bra straps
- restriction of physical activity due to the size and weight of your breasts
- dissatisfaction or self-consciousness about the largeness of your breasts
Breast Reduction can be performed at any age, but we usually recommend waiting until breast development has stopped.
Types of Anesthesia
Breast reduction will be performed in Bangkok Phuket Hospital or Phuket International Hospital and under general anesthesia, which means that patient will sleep through the operation.
Pre-operative care
The patient will be admitted to the hospital on the day of surgery. The patient needs to empty her stomach at least 6-8 hours before operation. If patient uses Aspirin or any anticoagulant drugs regularly, she has to stop at least 1 week before operation. Chest x-ray, EKG, routine blood tests will be done on the admission day. If the patient has any allergy history, she has to inform the doctor before surgery. The patient is admitted two nights in the private room after operation and the patient can be discharged from the hospital in the next day.
The Surgery
Techniques for breast reduction is superomedial pedical, vertical scar mammoplasty technique (The technique of Elizabeth J. Hall-Findlay). The patient will have scar around the areolar and one vertical scar down from the areolar to the lower fold of the breast (if necessary some case may have short horizontal scar in the crease of the breast).
The surgeon removes excess glandular breast tissue, fat, and skin, and moves the nipple and areola into a new higher position, then brings the skin from both sides of the breast down and around the areola, shaping the new contour of the breast. The areola, which in large breasts usually has been stretched, also is reduced in size.
Stitches are usually located around the areola, in a vertical line extending downward, and along the lower crease of the breast in some case.
After Surgery
After surgery, patient will be wrapped in an elastic bandage. A small tube may be placed in each breast to drain off blood and fluids for the first day or two.
Patient may feel some pain for the first couple of days especially when patient move around or cough and some discomfort for a week or more. Surgeon will prescribe medications to lessen the pain.
The bandages will be removed a day or two after surgery, though patient will continue wearing the surgical bra around the clock for several weeks, until the swelling and bruising subside. The stitches will be removed in one to three weeks.
The procedure does leave noticeable, permanent scars, although they will be covered by your bra or bathing suit. The procedure can also leave you with slightly mismatched breasts or unevenly positioned nipples. Future breast-feeding may not be possible, since the surgery removes many of the milk ducts leading to the nipples.
